Levels of care tailored to individual needs
Parkcliffe’s “Bed and Breakfast” -style living arrangements afford residents a comfortable blend of privacy, safety and community, allowing them to maintain an optimal quality of life. Residents have space to call their own, while shared living rooms, dining areas and large community areas encourage interaction with fellow residents, guests, and caregivers.
Parkcliffe does a complete cognitive and functional assessment for every new resident. Ongoing evaluations are done periodically and as needed, to assure each individual receives the appropriate level of care.
Level I
Shared Living
Shared Living provides a safe and secure home setting for those who can live independently, but require occasional help with activities of daily living (ADLs), such as dressing, bathing, personal hygiene, mobility, eating and self-administration of medication.
Level II
Assisted Living
Assisted Living maintains a focus on independent living, while providing more on-going caregiver involvement for residents who require greater assistance with ADLs and personal needs than those in Shared Living.
Level III
Personalized Dementia Care
Personalized Dementia Care offers specialized assistance for those with more advanced neurological impairment. The activity programming, social services and personal care provided each resident is specially designed to meet unique individual needs and support the independence of the resident to the greatest degree possible.
Level IV
Advanced Care
Advanced Care is provided in a building specially designed to meet the needs of the most impaired resident. The experienced staff provides comfort care, medication administration, periodic skilled-nursing care (less than 24 hours per day), and end-of-life care.
